All dogs joining our pack need to have  completed the form below. Please fill out the form in as much detail as possible.

Pack walking takes place in public spaces, on and off leash. Please be honest and provide as much information as possible about your dogs behaviour. It will not affect whether or not you can use our services, but will give us a good idea about your dogs behaviour, how we can best service your needs and what precautions we need to take in order to keep your dog, ourselves, the public and their dogs, as well as other animals safe.
